Registration Options:
Registration may be made online or via mail, fax or scan.
You may register online at www.acmq.org.

You can fax, mail, or email a printed registration form with completed payment information to the Meeting Registrar.

Method of Payment:
A registration is not considered final until payment is received. Payment may be made by check or credit card.

Payment by Check: (All fees must be paid in U.S. Dollars. Payment must accompany each registration. ACMQ does not provide invoices.) Check must be drawn on U.S. bank and made payable to “ACMQ”. If a company or university makes payment, please make certain your name is written on the check stub or accompanying correspondence to ensure it is properly applied.

For registrants awaiting a company check, a credit card must be given to hold registration. If payment is not received by seven days prior to the meeting, the credit card payment will be processed.

Payment by Credit Card: The following credit cards are accepted: American Express, MasterCard and Visa.

Cancellation Policy:
Cancellations, in writing dated or postmarked on or before 3/3/2015, are subject to a $100 administrative charge. ACMQ will retain the full registration fee for cancellations made after 3/3/2015. Substitutions are welcome. Refunds will be mailed after the conference. Registrants who do not cancel by 3/13/2015 and who do not attend, are responsible for the full registration fee.

Refunds will be issued after the meeting via method of payment (credit/check). Registration fees for cancelled registrations cannot be applied to future ACMQ meetings. Registrants who do not cancel prior to the conference and who do not attend, will be responsible for the full registration fee.

Intellectual Property:
Unauthorized sharing of ISPE Meeting content via Webcast access through the sharing of user names and passwords or via alternative media (Flash Drive) through the sharing of said media is restricted by law and may subject the copyright infringer to substantial civil damages.

General Terms and Conditions:
Program subject to cancellation or change. If the program is cancelled, the only liability of the ACMQ Meeting will be to refund the registration fee paid. The ACMQ Meeting shall have no liability regarding travel or other costs. A registration form, submitted via fax, mail, email or online, constitutes a binding agreement between the parties.

Registration Binding Agreement:
Registration (whether online or by paper form) constitutes a contract and all terms and conditions are binding on the parties. In particular, the terms and conditions shall apply in the case of any credit/debit card dispute.

Consent to Use of Photographic Images:
Registration and attendance at, or participation in, ACMQ meetings and other activities constitutes an agreement by the registrant to ACMQ’s use and distribution (both now and in the future) of the registrant or attendee’s image or voice in photographs, videotapes, electronic reproductions and audiotapes of such events and activities.

Email Communication:
By registering for the Meeting, you are providing permission to receive emails, mailings and faxes related to the Meeting. If you wish to opt out of receiving any of these materials, you may do so by emailing acmq@acmq.org.
