Thank you to all speakers and moderators for agreeing to join our Medical Quality 2011 faculty!

The speaker agreement and related documents can be completed electronically and returned as e-mail attachments, or can be printed and completed by hand, then returned by fax to 301-656-0989 or by mail to ACMQ, 5272 River Road, Suite 630, Bethesda, MD 20864.

Please note that deadlines for MOC questions have changed. Your deadlines for submitting materials to us are as follows:

August 31, 2010	- Brief bio sketch
October 31, 2010	- Speaker Agreement Form, including Disclosure of Interest Form and Speaker Release Form - Presentation abstract (not more than 200 words) - Learning objectives (3-5)
November 30, 2010	- Maintenance of certification questions (5 questions per 90-minute session)
January 15, 2011	- Presentation materials: PowerPoint slides or text document
May 1, 2011	- Expense reimbursement form
