Goal: To equip providers with the knowledge, skills, and attitudes necessary to provide compassionate, comprehensive, and patient-centered care for older adults that optimizes their health, function, and quality of life.
Learning Objectives:

Describe the core principles of age-friendly care: Articulate the key elements that differentiate age-friendly care from standard medical care, including focusing on what matters to the patient, respecting autonomy and dignity, promoting independence and function, and providing person-centered care.

Apply a structured approach to geriatric and palliative care: Demonstrate the ability to interpret data on patients encompassing physical, cognitive, functional, and psychosocial domains, to identify individual patient needs and develop appropriate care plans.

Implement communication strategies appropriate for older adults: Practice effective communication techniques tailored to the needs of older adults, including those with cognitive impairment or sensory deficits, ensuring clear understanding and shared decision-making.

Discuss the importance of interprofessional collaboration in geriatric care: Explain the roles of various healthcare professionals involved in the care of older adults and articulate the value of teamwork in optimizing patient outcomes.

Develop an age-friendly approach to prescribing medications: Apply principles of safe and effective medication management for older adults, including considering age-related physiological changes, drug interactions, and deprescribing when appropriate.
