The Aging Revolution

Goal: To equip providers with the knowledge, skills, and attitudes necessary to provide compassionate, comprehensive, and patient-centered care for older adults that optimizes their health, function, and quality of life.

Learning Objectives:  

  • Describe the core principles of age-friendly care: Articulate the key elements that differentiate age-friendly care from standard medical care, including focusing on what matters to the patient, respecting autonomy and dignity, promoting independence and function, and providing person-centered care.

  • Apply a structured approach to geriatric and palliative care: Demonstrate the ability to interpret data on patients encompassing physical, cognitive, functional, and psychosocial domains, to identify individual patient needs and develop appropriate care plans.

  • Implement communication strategies appropriate for older adults: Practice effective communication techniques tailored to the needs of older adults, including those with cognitive impairment or sensory deficits, ensuring clear understanding and shared decision-making.

  • Discuss the importance of interprofessional collaboration in geriatric care: Explain the roles of various healthcare professionals involved in the care of older adults and articulate the value of teamwork in optimizing patient outcomes.

  • Develop an age-friendly approach to prescribing medications: Apply principles of safe and effective medication management for older adults, including considering age-related physiological changes, drug interactions, and deprescribing when appropriate.
